Jika diantara kalian yang saat ini ingin menggunakan Kartu BPJS atau sekarang Kartu Sehat Indonesia, ingin menggunakan tapi hilang, tenang guys, bisa diurus dengan syarat yang cukup mudah asalkan kalian sabar antri.
Sebelum itu kalian harus datang ke kantor BPJS setempat kebetulan saat ini domisili saya di Yogyakarta,jadi yang ada di wilayah Yogyakarta ke Kantor yang di Jalan Gedong Kuning
Oke langsung saja berikut syarat-syaratnya
- Surat keterangan hilang, bisa dibuat dari kantor polisi atau bisa dibuat di Kantor BPJS,saya buat di Kantor BPJS tinggal isi form dan ditempel meterai Rp.6000,-
- Fotocopy Kartu Keluarga
- Fotocopy KTP
Yup itu saja syarat yang dibutuhkan, pastikan kalian mengisi form dengan lengkap,langkah selanjutnya ke bagian verifikator, tunggu panggilan sesuai dengan nomor antrian, dari sini jika berkas sudah lengkap kalian diberikan nomor antrian lagi, disini kalian harus sabar tunggu panggilan yaa
kartu baru langsung bisa jadi hari itu juga..
